Form settings
The following settings can be configured in the Form section of your popup or inline form.
| Setting | Description |
|---|---|
| Title | The title of the form, where you can provide an incentive or a call to action to submit a response. Maximum of 50 characters. |
| Content | The text displayed below the title. Consider offering an incentive for signing up and keeping the message simple. Maximum of 100 characters. |
| Fields | The information to be captured by the form. Forms have a limit of 31 fields per form. Learn more about managing form inputs. |
| Marketing consent | You can capture email and SMS marketing consent by selecting either of the following settings:
For email marketing consent, the email field is required on the form. For SMS consent, the phone field is required on the form. If you use double-opt-in for SMS, then the customer's smsMarketingConsent state will be set to pending. Your SMS partner is responsible for confirming opt-in with the number. After receiving confirmation, your SMS partner needs to update the status from pending to confirmed. |
| Button label | The text displayed on the submit button. The button should have a clear call to action, such as "Sign Up", "Subscribe", "Submit", or "Get my discount". Maximum of 25 characters. |
| Consent disclaimer (optional) | A disclaimer that includes information about how you use customer data. For example, you can explain that by submitting their email address, customers agree to receive marketing emails. Consider adding links to your store's terms and conditions or privacy policy. |
| Sign in with Shop | When your store has Sign in with Shop activated and your form requires an email or phone number, a Claim with Shop button displays on the form for customers. This option is turned on by default for eligible forms. You can turn off Sign in with Shop on a form. |
Custom fields
When you create a custom input field on a form, you can store it as a local field, or a customer or company metafield.
When you add a custom field to your form, you need to give the corresponding field a name. Optionally, you can set specific requirements for each field. Learn more about managing form fields.
Local fields
Fields stored locally are used only on the specific form that you add it to. To reuse a local field, you can duplicate an existing form that uses that field.
You can change a locally stored field to a customer or company metafield at any time by checking Also store as customer metafield. However, only data submitted after the date that you changed the local field to a metafield is stored in the metafield.
Customer or company metafields
Fields stored as customer or company metafields are saved in the form field picker. If you have 20 or less customer or company fields saved, then these fields are visible and available for reuse on other forms. Only currently pinned metafields can be reused. If you have more than 20 customer or company metafields, then you can manage your pinned metafields.
After you save a custom input field as a metafield, the Also store as customer metafield checkbox no longer displays, and it can't be changed to a local field. If you edit a metafield, then Forms updates all forms that use that metafield to reflect those changes.
After you store a form field as a metafield, it's saved in the Custom data settings of your Shopify admin, in the Customers or Companies section. You can use this metafield data to assess B2B company requests or to create customer segments.
If a customer fills in a field that's already linked to their customer profile more than one time, then their customer profile displays metafield data only from the most recent form submission.

A custom field can be any of the following data types:

Manage form fields
By default, forms capture the customer's first name and email address. In the Fields section of your form, you can add or remove fields to customize the information that you collect from customers.
For example, if you want to create a field to capture the shoe size of your customers, then you can select the Number field and name the metafield Shoe Size. You can then set the field to accept values from 1-16.
Steps:
- Follow the steps to create a form.
- In the Fields section, take any of the following steps to manage your form fields:
- To add a new field, click Add new field. Select and configure the field that you want to add, and then click Done.
- To remove a field, click the removal icon (⊖) next to the field's name.
- To edit the details of a field, such as whether the field is required to complete the form, click the field's name. After you make your changes, and then click Done.
- Configure your other form settings, and then click Save.
