Create forms

You can create forms to help you grow your business in the following ways:

  • Prompt customers to join your email marketing list
  • Intake wholesale retailers using a company account request form
  • Collect data about your customers that's important to your business
  • Create contact us forms for support inquiries

Forms can be customized using the editor to match your online store and capture customer information in a privacy-compliant way. Before you create a form, review the various form settings available to each different type of form.

You can display multiple popup forms, inline forms, or both types at the same time on your store.

Considerations for creating Shopify Forms

Review the following considerations for creating Shopify Forms:

  • Forms display on your store only when they've been activated.
  • You can have up to 25 forms per store.
  • Multi-step forms aren't supported.
  • Conditional logic isn't supported on forms.
  • When an existing customer submits a form, their email and phone number on file aren't modified, even when the form includes a different email or phone number. Other fields, such as marketing consent, also might not be updated. To change an existing customer's email or phone number, edit their profile directly in your Shopify admin.

Form types

You can create either a popup or inline form. Review the differences between popup and inline forms, and requirements to display them.

You can create a popup form to display on your store from the Forms app in your Shopify admin. After you create and activate a popup form, it automatically displays on your store.

Take the following steps to create a popup form and add it to your store:

Step 1: Turn on the Forms app embed for your theme

If you're creating a popup form for the first time, then you need to turn on the Forms app embed for your theme. As long as the Forms theme app embeds setting is toggled on, all active popup forms display on your online store.

If you've already turned on the Forms app embed setting, then proceed to Step 2: Create your popup form.

Steps:

  1. From your Shopify admin, go to Online Store > Themes.

  2. In your active theme, click Edit theme.

  3. In the theme editor sidebar, click the App embeds icon.

  4. Activate the Forms toggle.

  5. Click Save.

Step 2: Create a popup form

  1. From your Shopify admin, go to Apps > Forms.

  2. Click Create form.

  3. Select Popup form.

  4. Enter a name for the form, and then click Create.

  5. In the Popup display settings, do either of the following:

    • To create a floating popup form, select Floating, and then select a position on your store from the drop-down menu.
    • To create an overlay popup form, select Overlay, and then use the color picker to set an overlay color.
  6. Configure the following parts of your form:

  7. Optional: To create a customer tag after a customer submits your form, in the Tags field, enter a tag to select or create a new tag.

  8. Optional: If you want to receive email notifications each time the form is submitted, then in the Email notifications section, select Send an email for each submission to this form.

  9. Optional: To manage the automations and workflows associated with the form, in the Automations and workflows section, click Manage next to any workflow.

  10. Click Save.

Step 3: Translate your form to additional languages (Optional)

You can add translations in supported languages to your form. The steps to translate your form depend on the translation method that you want to use, and whether you've already added additional languages to your store.

If a translation doesn't have auto-translate available, then an "Auto-translation not available for this language" message displays.

Steps:

  1. In the Languages section of your form, do either of the following:
    • If you've already published additional languages to your store and they display in the Published section, then proceed to the next step.
    • If no available languages display in the Published or Unpublished section, then you first need to add a new language to your store:
      1. Click language settings.
      2. On the Languages page, click Add a language.
      3. Select a language from the drop-down menu, and then assign it to a domain.
      4. Click Done.
      5. Click <-- Forms to return to Forms, and then open the form that you want to continue editing.
  2. Depending on your preference and the translation method that's available, do either of the following:
    • Use auto-translate:
      1. Click Translate next to the language that you want to translate. If you have more than one language that you want to translate to, then you can select Auto-translate all.
      2. Click Review to preview your translations.
      3. Click Save.
    • Add your own translations manually:
      1. Click Translate next to the language that you want to translate.
      2. In each field in the column of the language you want to translate to, add your translation.
      3. Click Save.

Step 4: Preview and activate your popup form

After you create a form, its status is Inactive by default. Before activating your form, you can use the preview to confirm how each part of the form will display to customers.

When you're ready to display your form on your online store, click Turn form on. After you turn the form on, its status displays as Active.

Create and add an inline form to your store

You can create an inline form to add a form to a new page on your store, or embed the form on an existing page on your store. Take the following steps to create an inline form and add it to your store:

Step 1: Create an inline form

  1. From your Shopify admin, go to Apps > Forms.

  2. Click Create form.

  3. Select Inline form.

  4. Enter a name for the form, and then click Create.

  5. Configure the following parts of your form:

  6. Optional: To create a customer tag after a customer submits your form, in the Tags field, enter a tag to select or create a new tag.

  7. Optional: If you want to receive email notifications each time the form is submitted, then in the Email notifications section, select Send an email for each submission to this form.

  8. Optional: To manage the automations and workflows associated with the form, in the Automations and workflows section, click Manage next to any workflow.

  9. Click Save.

Step 3: Choose the placement for your inline form

Inline forms can be displayed on either a new landing page or an existing page template. Learn more about inline form placement.

After you save your form changes, configure either of the following placement options depending how you want your form to display on your store:

Add a new page to your online store for your form

When you create a landing page from the Forms app, you create a new page template and generate a new page using that template with a customizable handle, for example: example.com/pages/your-custom-form-URL.

Steps:

  1. From your Shopify admin, go to Apps > Forms.

  2. Click the inline form that you want to update placement for.

  3. In the Placement section of the form settings, click Create landing page.

  4. In the dialog, enter a title and URL for the landing page, and then click Save. A new page is created and added to the Pages list in your Shopify admin.

  5. Optional: To update the page title, content, or handle for your landing page, click Edit page. You can also make changes from the Pages list in the admin or by editing the page template in the theme editor at any time.

Add your form to an existing page template

You can add an inline form to an existing page template as a block in the theme editor by using the Form ID associated with your form. A form ID is a 6-digit number that represents a specific form in your store. The inline form displays on all pages that use the template you select.

  1. After you save your changes, in the Placement section of your form settings, select Existing page.
  2. Click the clipboard icon to copy the Form ID to your clipboard.
  3. Click Go to theme editor.
  4. In the Template section, add the inline form to your page template:
    1. Click ⊕ Add section.
    2. Select the Apps tab.
    3. Select Forms.
    4. In the block settings, paste the copied Form ID into the Form ID field.
    5. Optional: Adjust the colors, style, and layout settings of the block.
  5. Click Save.

Step 4: Preview and activate your inline form

After you create a form, its status is Inactive by default. Before activating your form, you can use the preview to confirm how each part of the form will display to customers.

When you're ready to display your form on your online store, click Turn form on. After you turn the form on, its status displays as Active.

Duplicate an existing form

You can duplicate a form to create a new form that includes all of the fields and settings of an existing form, with the exception of translations and inline form placement.

After you duplicate a form, the new form is set to Inactive by default. Learn how to activate a form.

Steps:

  1. From your Shopify admin, go to Apps > Forms.

  2. Click Horizontal menu for the form that you want to duplicate.

  3. From the drop-down menu, select Duplicate form.

  4. Enter a name for your new duplicate form.

  5. Edit any form settings that you want to change, and then click Save.

  6. Click Turn form on to display the form on your store.
